How to apply for Human Subjects Review Committee approval or exemption

To apply for HSRC approval or exemption please

1. Review the list of potential risks faced by subjects in SoTL studies
2. Complete the brief Human Subjects Review Committee application form
3. Develop an Informed Consent form for your prospective subjects (both Informed Consent guidelines and a template for this form are available)
4. Develop a Debriefing Form (Debriefing Form Template available)
5. Email your application form, Informed Consent form and Debriefing Form to the Human Subjects Review Committee chair for review
